Messed up with my partitions

hello,

think messed myself disks try find out there. :p

had multiple partitions, mac (yosemite) / windows, , tried install ubuntu third partition. dunno did after couldn't boot on windows anymore decided wipe , have fresh & clean install mac partition.

tried use bootcamp in order remove broken windows partition. unfortunately, did have option install new bootcamp partition. therefore, jump'd on first solution found on internet, delete partition through disk utility , partition , "-" bootcamp partition.

on, have in disk utility (in debug mode) :

i'd clean install, macintosh hd partition. don't know how... it's old windows partition size (40gb) lost.

i think problems started yosemite install. turns yosemite partition core storage volume cannot manipulated in disk utility. can see @ bottom there in disk util still have core storage on (that logical volume note there).

open terminal , enter line below tell me output , can try , undo this.

code:
diskutil cs list
